New Milford Medical Group is a dynamic multi-specialty medical practice with a long history of service. Started by Internist Dr. Harold Kamm in 1986, our vision from the beginning was to provide outstanding medical care to the people of the greater New Milford Community in a personal and cost effective manner. Along the way, we added Cardiologists and Family Practice Physicians as well as Physician Assistants and a host of ancillary testing including nuclear stress tests, echocardiography, vascular imaging and sleep medicine services within our practice.
As an independent medical practice for 32 years, our mission has never waivered. We remain intensely focused on delivering a premier patient experience within the context of cutting edge, cost effective and personal medicine. We strive to build loyalty and trust in our patients. Most of all, we want our patients to trust that we can and will help them with their medical needs and that we have our eye on the ball in regard to optimizing their long-term health through prevention.
To continue to be a relevant and vital care provider in our community, we must continue to grow and change. We are proud that our practice has been recognized by the National Committee for Quality Assurance as a Level 3 Patient Centered Medical Home. Level 3 is the maximum possible achievement and the standards for it are very high. It is the result of our ongoing commitment to offer the very highest level of health care to our patients.
A Patient-Centered Medical Home ("PCMH") is a team-based model of care led by a personal physician who provides continuous and coordinated care to maximize health outcomes. The PCMH practice is responsible for providing, and coordinating, a patient's health care needs and appropriately arranging care with other qualified professionals. This includes the provision of preventive services, treatment of acute and chronic illness, and assistance with end-of-life issues.
It is a model of practice in which a team of health professionals, coordinated by a personal physician, work collaboratively to provide high levels of care, access and communication, to both patients and their families.
In order to win recognition as a Level 3 Patient Centered Medical Home, a medical practice must demonstrate that it has met, or exceeded, the standards that have been developed by the National Committee for Quality Assurance for this purpose. New Milford Medical Group scored over 90% compliance with these standards.
